CU Anschutz Student Organization Advisor Registration/Renewal Form


This form is to be completed annually. The signature of the organization advisor is required before submission. Organization President will provide a signature and documents in another form.

Taking registration for the 24-25 AY.

Signature of Advisor for Student Organization

By signing this form, you, as the Student Organization Advisor agrees to be responsible for compliance with State, University, and CU Anschutz campus rules and regulations affecting the conduct of student group business on campus. In addition it is agreed that:

1. The Student Organization Advisor agrees to facilitate access to the following University services:

Procurement Services
Travel Accounting
Legal Counsel
Compensation Services
Staff and Faculty Personnel
Name and Marks of the University of Colorado
State of Colorado and Federal Tax-Exempt Status
Governmental Status Benefits

2. Understand that the student organization is not protected by the University from being liable for their debts, violations of the law or contractual obligations made outside of the University Procurement Services.

3. Agrees to ensure that the student organization account(s) on campus maintains a positive balance at all times. The balance of the student organization's account(s) on campus must be sufficient to cover all expenses and encumbered expenses related to any activity sponsored by the student organization.

4. The student organization shall not discriminate on the basis of race, color, religion, national origin, disability, gender, citizenship, sexual orientation or age when determining its membership. Notwithstanding these requirements, a club sport or athletic activity in which the student organization chooses to participate may restrict team membership based on gender where selection for such teams is based upon competitive athletic skill or the activity involved is a contact sport. The student organization shall consider the needs of persons with disabilities in planning and conducting membership recruitment and the activities of the student organization. The actual practices and operations of the student organization are also relevant.

5. The student organization also assures that its group membership has as its majority, students who are paying the CU Anschutz activity fee. Faculty, staff and friends of CU Anschutz may also participate, but are not allowed to hold officer positions within the student organization. Faculty, staff and friends may also be required to pay additional fees (at least that of the CU Anschutz student activity fee, any additional amount shall be set by the officers of the Affiliated Student Organization) in order to participate.


It is understood and agreed upon by the advisor that failure of the above-named student organization to maintain a positive balance sufficient to cover all expenses/encumbered expenses will be sufficient cause for the University to terminate this agreement unilaterally and without notice.

Failure to comply with any of these stipulations or related CU Anschutz Policies and Procedures may result in this Organization’s status as ‘Affiliated’ being unilaterally revoked by the Office of Campus Student Services and or the CU Anschutz administration.

Next Steps:

-Your renewal form will be forwarded to the Student Engagement Office to be reviewed.

-All submissions will receive an email from the Student Engagement Office informing them if they are initially approved or denied.

- If initially approved, the President and Advisor must complete the online training if they haven't previously completed it. 

- Once completed, your organization is approved and can benefit from extra funding and event support. Affiliated SO with SDM and SOP might have additional training and will be forwarded to the appropriate parties.
