LInQS Fellowship Program Application Form

The objective of the Leaders in Informatics, Quality and Systems (LInQS) Fellowship Program is to develop CUSOM Department of Medicine fellows and faculty into future leaders with successful academic careers in the areas of quality improvement (QI), clinical system design, clinical informatics, and/or patient safety.  The LInQS Fellowship Program will provide didactics, longitudinal mentorship and hands-on training from faculty mentors with diverse expertise to support trainees and faculty.


Program Description:

The didactic portion of the program will include participation in the Quality Safety Academy and the Institute for Healthcare Quality, Safety & Efficiency (IHQSE) Foundations in Healthcare Leadership Workshop in addition to the faculty-led lecture series.  Participants will attend regular work-in-progress meetings and be able to tailor their experience to meet their career goals.  Divisions will support the participants through protecting a percentage of their time to dedicate to the program (see below).  

Examples of deliverables for participants at the end of the 2-year program (Must complete at least 3):

  • (Required) Lead a QI project that has significant clinical impact on Division
  • (Required) Publish QI project in peer review journal, submit project to a peer review journal, and/or present at local/national conference
  • Lead and teach quality improvement, patient safety and/or clinical informatics to trainees
  • Facilitate DOM Systems Improvement Conference
  • Participate in select hospital or health system committee meetings over the course of the fellowship
  • Submit a grant for intramural or extramural funding

Program Eligibility: 

  • Program open to DOM fellows and junior faculty (physicians and advanced practice providers)
  • Obtain a letter of support from Division Head or Program Director outlining commitment towards protected time (i.e. reduction of clinical duties) for fellows (75%) and junior faculty (minimum of 10% reduction in clinical time or one-half day per week).   

APPLICATIONS WILL BE ACCEPTED UNTIL February 29, 2024.  Cohort 6 (AY2024-2026) will begin July 1, 2024.

Letter of Support

Divisions will support participants in the LInQS Fellowship Program by protecting a percentage of their time.  LInQS Fellowship Program participants will attend the following didactics: 

  • Quality Safety Academy (12 hours) - Meets in the Fall 2024 (4 sessions) 
  • LInQS Fellowship Program faculty lecture series & work-in-progress meetings (46 hours) - Meets every 1st & 3rd Wednesday from 3-5p beginning July 2024 (1 year)
  • Bi-weekly mentor meetings with LInQS Mentor (2 years)
  • Institute for Healthcare Quality, Safety & Efficiency Foundations in Healthcare Leadership (28 hours) - Meets 3.5 days in beginning Spring 2025 + 1 year of monthly mentorship meetings 
    • The cost of this activity is $4,000 per applicant.  Divisions must document their commitment to cover this cost in the letter of support.

In addition to the didactic portion, LInQS Fellowship Program participants will complete at least three of the following program deliverables: 

  • (Required) Lead a QI project that has significant clinical impact on Division
  • (Required) Publish QI project in peer review journal and/or present at local/national conference
  • Lead and teach quality improvement, patient safety and/or clinical informatics to trainees
  • Facilitate DOM Systems Improvement Conference
  • Participate in select hospital or health system committee meetings over the course of the fellowship
  • Submit a grant for intramural or extramural funding

The amount of protected time needed will vary based on the applicant's position.  Fellows are expected to dedicate 75% FTE and junior faculty (physicians and advanced practice providers) are expected to dedicate 10% FTE or one-half day per week.  Divisions must agree to decrease the applicants clinical time by 10% for faculty applicants.  The didactic portion will be the same for all LInQS Fellowship Program participants, but the program deliverables will vary based on % FTE commitment from divisions.  Using the link provided, you may include a letter of support from your Division Head or Program Director outlining his/her commitment towards protected time for the program.
