CAP Certificate Completion Form

Please submit this form if you have completed the requirements for a graduate certificate offered by the College of Architecture and Planning (whether you are a non-degree certificate student or a degree-seeking student earning the certificate in addition to your degree). Completing this form allows the College of Architecture and Planning to approve the awarding of your certificate. You will not receive a physical certificate but it will be noted on your transcript.

Please note: If you do not complete this form, you are at risk of not having your certificate awarded on your transcript. The deadline for submitting this form corresponds with the graduation application deadline for the semester in which you are completing the certificate. Please visit the CU Denver Academic Calendar for these dates.

This form does not replace the required graduation application for degree-seeking students. If you are also completing a degree program in the current semester, you must follow the graduation procedures for your college.
