Alpha Phi Sigma Honor Regalia Form

This form should only be completed by students who are current Alpha Phi Sigma members, are graduating this current semester and would like to purchase Alpha Phi Sigma Honor Regalia.

All orders will be mailed directly to the address you provide on your paperwork. Please be sure to include your mailing address, and opt into shipping and handling.

Do not submit your payment, you will be billed directly by Alpha Phi Sigma National headquarters.

Not Currently an Alpha Phi Sigma Member: If you are planning to graduate this semester and want to wear APS regalia and you are not already a member, you must first complete the Student Membership Application, then the Honor Regalia Form.

Your next step is to download, complete and then upload the Honor Regalia Order Form.

Once you upload and submit this form, it will be routed to the Alpha Phi Sigma Chapter Advisor and then to Alpha Phi Sigma headquarters for final processing.

If, on your membership application you indicated that you were an Online Only student, your Honor Regalia will be shipped directly to the address you provide on the form.

If you did not indicate that you were an Online Only student, your Honor Regalia will be shipped to the University and you will need to pick-it up on-campus at the School of Public Affairs.
